Create a smart form
Create a smart form and configure global limits, field mappings, preset items, and anti-abuse controls.
Overview
Use the form designer to collect data for Sales Leads, Accounts, or Custom Objects. Configure required, read-only, hidden, and default values. Add a Verification Code to prevent abusive submissions.
Before you begin
[!IMPORTANT] - You need configuration permission for Smart Forms. - Required object fields are always required on the form.
Procedure
1. Create a form

Go to Setup > Object Management > Smart Forms.

Click New.
2. Configure global settings
- Form Name: Enter the title.
- Form Description: Enter visitor instructions.
- Expiration Date: Set when the link and QR code expire.
- Each User Can Submit Only Once: Prevent duplicate submissions from one visitor.
3. Configure fields and layout
- Drag fields from the left panel to the form designer.
- Configure each field in the property panel:
- Display Name: Set a visitor-facing alias.
- Required/Read-Only/Hidden: Hide helper fields and set default values when needed.
- Preset Item: Hide a field from visitors and let employees preset it in distributed links.
4. Add anti-abuse controls
- Drag the CAPTCHA component onto the form.

Visitors must complete the CAPTCHA before submission.[!WARNING] If the form contains an Image or Attachment field, the system requires CAPTCHA even when you do not add the component.
- Click Save.
Maintenance note
New required fields on the source object do not automatically appear on published forms. Edit each affected form and add the field to prevent synchronization failures.
